See Which Patients to Contact First
Clinigence aggregates clinical data across all providers, EHR systems, and care settings to allow you to see the full picture of each patient. You can choose which patients to contact based on their recent ED visits, chronic conditions, care gaps, and more. You can also prioritize your caseload based on the acuity or urgency of your patients’ needs.
For example, you can see which patients had the most ED visits over the past 12 months, sort them on based on the number of care gaps and quickly see whether they are due for a wellness visit, who their primary care provider (PCP) is and when they last visited.
Use Data to Move Patients
As you contact a patient you need to see a snapshot of her current status and needs. Clinigence shows you the patient’s most current vitals, chronic conditions, immunizations/screenings (as per CDC guidelines), and any care gaps relative to applicable clinical guidelines and quality measures. You can use this information to engage the patient and get her to move towards better compliance with her care plan.

Record Activities to Finish
In order to track the patient’s progress, you must take notes of your interaction with her. Clinigence enables you to keep track of different types of care management activities, such as patient education, outreach, medication review, etc. Your organization can also define its own custom care activity types. Each activity is logged as part of the patient record. You can add free notes and record the time spent. The latter may be especially useful for activities covered under Medicare’s Chronic Care Management (CCM) or similar billable services.
