CPC+ is a multi-payer program offering as much as $100 per patient per month for non-visit based care management services - many of which you may already be delivering to your patients.

Congratulations: If your practice is in one of the 4 regions (Louisiana, Nebraska, North Dakota and NY's Greater Buffalo Area) selected for Round 2 of the Comprehensive Primary Care Plus (CPC+) initiative starting in 2018, you are now eligible to apply (until July 13, 2017).
If you are already participating in CPC+, you know that the goal of CPC+ is to improve the quality of care patients receive, improve patients' health, and spend healthcare dollars more wisely. CPC+ also offers substantial performance-based incentives and exempts participants from the Merit-based Incentive Payment System (MIPS) - the 2017 successor of the Physician Quality Reporting System (PQRS) and Meaningful Use (MU) programs.
Why You Need Health IT
To be eligible, CPC+ requires the use of health IT in your practice. Including:
- Certified EHR technology (CEHRT).
- EHR-based reporting of certain clinical quality measures at the practice level.
- The ability to develop and optimize a set of health IT functions that work for the practice.
Comprehensive primary care requires efficient, advanced health IT to support its population-health focus and team-based structure.
